Report Intake Form

The Colorado Association of Libraries seeks to provide an environment in which diverse participants may learn, network, and enjoy the company of colleagues in an environment of mutual human respect. We recognize a shared responsibility to create and hold that environment for the benefit of all.


Do not use this form to report events presenting an immediate threat to life or property. Reports submitted through this form may not receive an immediate response. If you require emergency assistance, please contact your local authorities.


Report Handling Process:

Reports will be received by the CAL Executive Committee, who will appoint members of a Review Committee. 

Review Committee Composition:

Review Committee members may be made up of CAL Committee and Interest Group members, but shall participate and hold active membership within CAL.

The Review Committee shall not be made up of members who have any involvement with the parties named or inferred in the report. Executive Committee members shall recuse themselves from the Review Committee appointment panel to minimize conflicts of interest. 

The Review Committee will make every effort to follow the Reporting Policy timeline:

  • Acknowledgment: A representative of the Executive Committee shall acknowledge receipt of a report within 2-3 business days.
  • Review: The Executive Committee will thoroughly review your report. This may involve contacting you for further information.
  • Request for more information: The Committee may request additional information, such as whether the parties have attempted to discuss the matter before escalating, or if the matter is still ongoing.
  • Response: Either committee will aim to provide a written response to a report within 10 business days of receiving it. If the investigation requires more time, a notice of the expected timeline will be provided to the reportee.
  • Resolution: The committees will make every effort to resolve a report fairly and to satisfaction.
  • Escalation (if applicable): If a resolution is not satisfactory, reportees may escalate their report to the CAL Board of Directors by contacting them at cal@cal-webs.org. The escalation process will begin with the Executive Director and be presented to the Board.
  • Record Keeping: CAL will maintain records of all reports received and the actions taken for no longer than three years.


CAL will strive to adhere to the timeframes outlined in this policy. However, in complex cases, it may take longer to investigate and resolve a report. The Executive or Review Committee will inform the reportee of any delays.

Confidentiality:

All reports will be treated confidentially. Information shared in the report will only be used for the purpose of investigating and resolving the reported issue within the Reviewing Committee.

Any matters shared outside the Committee that break confidentiality will be addressed immediately by the Executive Committee or the Board of Directors. 


Report Contents

A report should include the following information:

  1. The name(s) and affiliation(s) of the individual(s) submitting the allegation, the victim(s), and the name of individual(s) alleged to have engaged in the prohibited behavior(s), if known. If not known, then any possible identifying information should be provided.
  2. A description of the allegation that includes the date (or range of dates) and circumstances of the alleged misconduct, and the name(s) and affiliation(s) of all parties involved, including any witnesses.
  3. Any documents or other relevant items (e.g., written or digital correspondence, physical evidence, etc.) with an annotation detailing how the item relates to the allegation.
  4. An explanation of the type of prohibited behavior(s) as defined in this Code of Conduct that are being alleged.
  5. A statement explaining any real or reasonably perceived conflict(s) of interest related to any parties named in the allegation and/or any member(s) of the CAL Board of Directors or leadership. This is to ensure a fair and unbiased process. A conflict of interest does not preclude the filing of an allegation.

Reports may be returned if the above information is not included.
